  1. Read the Participant Information Sheet

Before you register to be interviewed for this research, you need to read the Participant Information Sheet.

2. Register to be Interviewed

Fill in the registration form by clicking the button below.

This tells us a little bit about you, and ensures we know who - and what communities - are being represented in this mahi.

It’s also an opportunity for you to share anything about yourself or your journey with us before the interview so that we can make this a comfortable experience for you.


3. Complete the consent form

We must have your signed/completed consent form before we are able to book your interview.

This is available in a printable easy-read version, and an online version below.

4. Booking your interview

Once we have received your registration form, and signed consent form, one of our Changing Minds whānau will email you to book your interview.

We will book this for a time that suits you.


5. After your interview

After your interview please complete and return the koha form below to us by emailing insights@changingminds.org.nz
